Do not treat an online price table as a quotation
Do not assume that a visitor can use Japan’s public health insurance as an enrolled resident would. Confirm your own status and coverage. The amount charged can vary by facility, treatment, tests, hospitalization and support required. Ask for an estimate when it is safe and practical, but emergency treatment may begin before a final amount is known.

Contact the insurer when it is safe
Ask whether assistance, pre-authorization or a designated medical institution is required. Do not postpone emergency care to make this call.

Confirm the payment arrangement
Cashless or direct payment may be possible only when both the insurer and medical institution approve it. A card, logo or policy label alone does not guarantee cashless treatment.

Prepare for upfront payment
If direct payment is unavailable, you may need to pay the medical institution and submit a claim later. Keep an itemized bill, every receipt and the documents specified by the insurer.
Japan’s unpaid-medical-cost information system has operated since May 10, 2021. For applicable unpaid receivables arising on or after April 1, 2026, the Ministry of Health notice lowered the reporting threshold from ¥200,000 to ¥10,000. Information may be shared with the Immigration Services Agency and used in a future immigration examination.
This does not mean every unpaid bill automatically results in refusal of entry. It does mean that leaving without resolving a bill can have consequences. Contact the medical institution and insurer, confirm the amount and obtain written payment instructions for any remaining balance.
